This report describes a rare case of an 87-year-old woman presenting to a rural emergency department with ataxia. Throughout her admission, she developed areflexia and ophthalmoplegia consistent with a diagnosis of Miller Fisher syndrome. The patient underwent a rapid recovery after receiving treatment with intravenous immunoglobulin (IVIg) for a duration of five days and a two-month period of outpatient physical therapy.Agricultural workers are at a higher risk for skin cancer due to excessive amounts of sun exposure, poor sun safety practices, and limited access to healthcare. This study aimed to assess skin cancer history, sun protection practices, and skin cancer surveillance measures among Midwestern agricultural workers.
Individuals that attended the 2020 Sioux Falls Farm Show were asked to complete a 12-question survey on sun exposure, sun protective practices, and skin cancer surveillance. Survey responses were aggregated and analyzed using Microsoft Excel.
A total of 99 individuals participated in the study, of which 70 identified as agricultural workers. Most individuals reported working outdoors eight to 10 hours daily for 10-12 months of the year. Most participants reported poor sun protection habits, such as limited sunscreen and limited wide-brimmed hat use. A large number of participants reported they had previously received a professional skin check, although 33 percent reported never examining their ownstantial sun exposure, poor sun protection, and minimal self-skin examinations. Our findings may help guide future methods to increase skin cancer education, prevention, and surveillance in agricultural workers.It is estimated that 25-30 percent of known pregnancies result in early pregnancy loss, which can contribute to significant mental health issues for women. In a local health system, it was identified that there was no standard of care for mental health resources offered to patients experiencing a first trimester miscarriage. To create a model of care and a set of resources that the health system could offer, miscarriage patients nationwide were surveyed about resources and care they received or did not receive, and how that contributed to their satisfaction. This was accomplished with an online, anonymous survey distributed via social media. The survey had 687 responses, with 646 confirming a first trimester miscarriage. Only 13.0 percent (95 percent CI 0.10,0.16) of patients surveyed were offered mental health resources, while a majority desired to receive them. Additionally, only 28.4 percent (0.25,0.32) of patients had a provider well-check, something 84.4 percent (0.81,0.88) of women wanted. Overall, patients reported a mean satisfaction with their care and resources of 5.1 out of 10 (4.89,5.35) and 4.1 out of 10 (3.86,4.32) respectively. Using this data, the goal is to create a standard protocol that health systems can easily implement to improve miscarriage patient satisfaction.Uterine malformations occur early in embryogenesis due to abnormalities in development of paramesonephric (Mullerian) ducts. Meta-analysis demonstrates an association between response impulsivity and Juvenile Myoclonic Epilepsy (JME), a common genetic generalized epilepsy. Here, we test the hypotheses that trait impulsivity is (i) elevated in JME compared to controls; (ii) moderated by specific seizure characteristics; and (iii) associated with psychiatric adverse effects of antiepileptic drugs (AEDs).
322 participants with JME and 126 age and gender-matched controls completed the Barratt's Impulsiveness Scale (BIS-brief) alongside information on seizure history and AED use. We compared group BIS-brief scores and assessed associations of JME BIS-brief scores with seizure characteristics and AED adverse effects.
The mean BIS-brief score in JME was 18.1±4.4 compared with 16.2±4.1 in controls (P=0.0007). Elevated impulsivity was associated with male gender (P=0.027), frequent absence seizures (P=0.0004) and lack of morning predominance of myoclon-striatal and thalamocortical networks as a shared mechanism between seizures and impulsivity in JME. These findings warrant consideration of impulsivity as a distinct target of intervention, and as a stratifying factor for AED treatment in JME, and perhaps other types of epilepsy. The role of impulsivity in treatment adherence and psychosocial outcome requires further investigation.Although aberrant expression of N6-methyladenine (mA) methylation-related genes contribute to tumorigenesis in many solid tumors, the prognostic value of the mA-related genes and their correlation with clinicopathological features in gliomas need advanced study.
The clinical and sequencing data of 288 patients with glioma were extracted from Chinese Glioma Genome Atlas database. By univariate and multivariable Cox regression analysis, the mA-related prognostic genes were identified, and their correlation with clinicopathological features was further analysis. A nomogram was constructed by R software and the performance of it was assessed by calibration and time-dependent receiver operating characteristic curve.
Nine mA-related genes were identified as independent prognostic factors, which were mostly enriched in RNA splicing, regulation of immune response and vesicle-mediated transport. By expression value and regression coefficient of these genes, we constructed risk score of each patient, tients with glioma, and is associated with clinicopathological features. Moreover, the nomogram based on the nine m6 A-related genes signature and clinicopathological features had good efficacy in predicting the survival probability.
